If an employee/volunteer will be driving on behalf of the Diocese, their MVR(Motor Vehicle Report) must be approved. An approval expires after two years. If you will be driving your personal vehicle you must provide proof of the required 100,000/300,000 liability in your insurance policy by providing a copy of your insurance declaration page to the Office of Safe Environment/HR Dept. For use by anyone wishing to file a report of sexual misconduct by Church personnel or volunteers.VECHS Waiver Form
Must be signed and sent to the Office of Safe Environment/HR Dept. by all employees and volunteers that are fingerprinted for VECHS, DCF and/or summer camps, giving permission for a background check/fingerprinting to be completed. This form is not a background check form. A volunteer or employee must still complete the process through their own parish, school or diocesan site office.
